How fillers work

Hyaluronic acid fillers are gels that attract water and add volume where they're placed. Different fillers have different thicknesses: softer gels for lips and fine lines, firmer gels for lifting cheeks and defining the jaw.

Hyaluronic acid fillers vs. collagen stimulators

Hyaluronic acid fillers, like Juvederm, Restylane, RHA and Revanesse Versa, add volume the moment they're placed and can be dissolved if needed. Collagen stimulators work differently. Sculptra and Radiesse encourage your skin to build its own collagen, so results develop gradually and tend to last longer. Many treatment plans use both: hyaluronic acid for shape and definition, and a collagen stimulator for overall firmness.

Choosing the right filler

How long fillers last

Areas that move a lot, like the lips, break filler down faster. Your metabolism and activity level also play a role.

Before your appointment

• If your doctor says it's safe, pause optional supplements that can increase bruising, like fish oil, vitamin E and ibuprofen, for a few days beforehand. Never stop a prescribed medication without asking your doctor.
• Skip alcohol for 24 hours before treatment.
• Book at least two weeks before a big event so any swelling or bruising has time to settle.
• If you get cold sores and are having lip filler, let us know. An antiviral may be recommended.

What to expect at your appointment

Consultation. Your injector looks at your face as a whole, not just one area, and recommends a plan that keeps your features balanced.

Treatment. Filler is placed with a fine needle or a blunt cannula. Most appointments take 30 to 60 minutes.

Afterward. Mild swelling, tenderness or bruising can last a few days. Most people return to normal activities the same day.

Aftercare

• Avoid strenuous exercise, alcohol, saunas and hot yoga for 24 hours.
• Don't press, rub or massage the treated area unless your injector asks you to.
• Use a cold compress for swelling, and sleep with your head slightly raised the first night.
• Call us if you notice pain that's getting worse, skin color changes or anything that concerns you.

Is filler safe?

Dermal fillers are FDA-approved for specific areas and have a strong safety record when placed by trained, licensed injectors. Swelling, tenderness and bruising are common and temporary. Rare complications include infection, lumps and, very rarely, filler blocking a blood vessel, which is why injector training and anatomy knowledge matter so much. Every treatment at Cosmetica is overseen by our board-certified dermatologist, and we keep hyaluronidase on hand to dissolve hyaluronic acid filler if it's ever needed.

Who shouldn't get filler?

Filler isn't recommended during pregnancy or breastfeeding, over an active skin infection or cold sore, or if you're allergic to lidocaine or any filler ingredient. Your provider reviews your health history before treatment.

Results are visible right away and settle over about two weeks. Individual results vary.

How long do dermal fillers last?

Most hyaluronic acid fillers last between 6 and 18 months, depending on the product and the area treated. Cheek and chin filler usually lasts longer than lip filler.

Will I look overdone?

Good filler work shouldn't be obvious. Your injector will recommend the smallest amount that achieves your goal and build gradually if needed.

Does filler hurt?

Most fillers contain lidocaine, a local anesthetic, and we can apply numbing cream first. Most people feel pressure and mild pinching.

Can filler be dissolved?

Hyaluronic acid fillers can be dissolved with hyaluronidase if you ever want to reverse or adjust your results.

Is filler the same as Botox?

No. Filler adds volume under the skin, while Botox relaxes the muscles that cause expression lines. They're often used together.
